The botulinum toxin, know in the general public as botulinum toxin is particularly effective in treating wrinkles of the forehead, around the eyelids and the glabella, enclosed between the two eyebrows which tightens facial expression. The most common medicine known containing botulinum toxin is Botox®, produced by Allergan.

WHAT IS THE BOTULINUM TOXIN?

The botulinum toxin (Botox® – Dysport®) is a protein produced by a bacteria known as Clostridium botulinum which has the particular capability of blocking the neuromuscular transmission. Its use as a medicine began in 1960 by a group of ophthalmologist in the research level and in 1980 was finally used for the treatment of strabismus (crossed eyes) and for blepharomspams (involuntary forced blinking) both conditions generated by an abnormal muscle contraction. Since, more applications where discovered in the coming years including achalasia, sweeting and muscular spams due to cerebral palsy.
The cosmetic use of the botulinum toxin began in the late 1980’s for the treatment of the frown lines, since then its application in the dynamics of the face has spread importantly.

HOW DOES IT WORK?

The toxin is capable to prevent muscular contraction by blocking the neuro-muscular transmission at the synapses level. The body needs around four to six months to progressively restablish the synapses.

HOW LONG DOES THE EFFECT LAST FOR?

The treatment by Botox® or Dysport® can last from four to six months according to the amount of units that have been injected. It is important to highlight that the muscular contraction always returns with the same strengh.

WHAT ARE THE POSSIBLE SIDE EFFECTS

Bruising at the injection site, head each and migration of the product paralyzing or weakening neighbour muscles. Another side effects are listed at the Swissmedic site.

Wrinkles appear as part of the normal aging process. They result from the contraction of facial muscles associated to internal factors such as a reduction of the skin elasticity, reduced production of collagen and elastin and the type, texture and thickness of the skin. Some external factors also play an important role as sun exposure, alcohol and cigarets consumption.

 

Wrinkles are classified according to their depth in the skin:
Type I wrinkles are those which are only visible during the muscle activation.
Type II wrinkles are moderately visible without muscle activation.
Type III wrinkles are highly visible without muscle activation and almost represents an scar in the skin.

Treatment varies depending on the type and nature of the wrinkle. One of the leading treatments consists of ‘filling’ the wrinkles through an injection of a gel called ‘hyaluronic acid’. The effect is immediate and lasts for an average of four to six months.

What is the gel of hyaluronic acid?

It is a very large molecule which is widely distributed in the organism. Forms part of the extracellular matrix ‘as cement’ between the cells, as a lubricant in the joints and part of the content of the eye. It aspect is transparent and viscous.

The hyaluronic acid is degraded by an enzyme called hyluronidase. The average time to be fully degraded is around 6 months, however the time can be longer depending of the concentration of the product.

Adding the gel of  hyaluronic acid into the dermis have demonstrated to bring enormous benefits to the skin as hydration because of its hydrophilic properties and tightening due to the activation of fibroblast. It also induce collagen formation and normal skin function. These benefits make hyaluronic acid a very effective topical humectant.

The gel can be injected inside the superficial and deep dermis or under to dermis depending on the reached result.

The most common products that we use in our clinic are: Juvederm®, Voluma®, Volbella®, Belotero® and Restylane®.
